California Forklift Posters E-Verify Notice Alaska Workers' Compensation Form
Employers have the responsibility to comply with 29 CFR 1910.178 in order to ensure the safe operation of powered industrial trucks at their facility. The Alaska Workers' Compensation Act requires each employer having one or more employees in Alaska to obtain workers' compensation insurance, unless the employer has been approved as a self insurer. Generally, an employer who is covered by Alaska Workman's Compensation Insurance is not subject to civil liability or suit by the injured worker. This is one of the main reasons for an employer to carry Workers' Compensation Insurance.

Employers are required to provide employees with information about unemployment insurance if they have worked for an eligible employer during the 18 months prior and are currently unemployed or working part time. The Employment Security Law of Arizona establishes the eligibility requirements for the payment of unemployment insurance benefits in the State of Arizona.

All employers must provide information about pregnancy leave rights to their employees and post this information in a conspicuous place where employees tend to gather. Employers who provide employee handbooks must include information about pregnancy leave in the handbook.


An employer must provide notice to his/her employees of the right to request a California Family Rights Act leave and must post the notice in a conspicuous place or places where employees tend to congregate. If the employer publishes a handbook describing other kinds of personal or disability leaves and makes it available to his/her employees, the employer must include a description of California Family Rights Act (CFRA) leave in its next edition.

The Paid Family Leave law requires employers to provide the Paid Family Leave pamphlet only to new employees and employees who request leave to care for a seriously ill family member or bond with a new minor child. Employers are not required to provide the Paid Family Leave insurance claim forms to their employees.
All employers must distribute an information sheet on sexual harassment to all employees. An employer may either distribute this pamphlet or develop an equivalent document that meets the requirements of Government Code section 12950(b).
